Responsibilities As a Social Worker in this role, you will report clinically to the Professional Practice Chief of Social Work and administratively to the Manager of Clinical Operations. Your primary responsibilities will include conducting thorough psychosocial assessments of patients and their families, utilizing various assessment tools in line with professional standards and clinical policies. You will provide therapeutic clinical counseling services, including grief and bereavement support, ensuring that patients and families receive the emotional and practical assistance they need.
Collaboration is key; you will engage with patients, families, community resources, and interdisciplinary teams to optimize service delivery and care planning. Additionally, your role will involve creating and facilitating educational sessions and support groups while contributing to research initiatives aimed at enhancing service effectiveness.
Requirements To qualify for this position, you must hold a Master of Social Work (MSW) degree and possess relevant experience in clinical social work. A strong understanding of assessment techniques and therapeutic interventions is essential. You should excel in communication and collaboration skills, with a passion for improving patient care.
