Responsibilities **
As a Nurse Case Manager, you'll orchestrate complete care and support services for clients via virtual assessment, strategic planning, and continuous case oversight. Your role encompasses clinical evaluation, care coordination, patient advocacy, resource management, and educational support—all without requiring field visits. Key duties include maintaining compliance with regulatory college standards, customizing program elements to meet prescribing physician needs, conducting patient enrollment processes, and partnering with Field Nurse Case Managers.
You'll evaluate patient status, complete enrollment calls, analyze patient charts for coverage authorization, and provide comprehensive medication education. Additionally, you'll demonstrate patient-centered care principles, engage in collaborative treatment planning, and coordinate services according to established KPIs to ensure rapid access to therapies.

Requirements **
Candidates must hold current nursing credentials and demonstrate proficiency in clinical assessment skills, including medical history evaluation and symptom management capabilities. Strong documentation abilities are essential, along with expertise in patient assessment techniques and care plan development.
Experience with virtual care delivery, regulatory compliance knowledge, and familiarity with specialty medications preferred. Excellent communication skills and cultural competency are vital for this patient-focused role.
