Mobile Physiotherapist - Adult Home Visits, Toronto ON

**Overview**

VHA Home HealthCare, Ontario's leading not-for-profit home care organization, seeks a dedicated Physiotherapist to join our innovative House Calls Program. This 35-hour weekly position offers the opportunity to deliver specialized care directly to seniors' homes, supporting vulnerable adults who face challenges accessing traditional clinic-based services. As part of our interdisciplinary mobile healthcare initiative—a collaboration between primary care, SPRINT Senior Care, and VHA Home HealthCare—you'll work alongside physicians, nurse practitioners, social workers, and occupational therapists to enhance quality of life for cognitively, physically, and socially fragile seniors across Toronto.

Responsibilities **

Conduct comprehensive physiotherapy assessments using standardized evaluation techniques and professional practice guidelines established by VHA. Develop and execute targeted intervention plans based on thorough client evaluations and individual needs. Collaborate within our integrated healthcare team to provide holistic care management, including chronic disease support, acute care interventions, and safe hospital-to-home transitions.

Facilitate improved health outcomes through evidence-based treatment approaches while maintaining VHA's commitment to continuous quality improvement. Foster positive relationships with clients, families, and interdisciplinary team members to ensure exceptional care delivery.

Requirements **

Registered Physiotherapist in Ontario with current professional standing.

Experience working with senior populations and managing complex health conditions preferred. Strong assessment and intervention skills with diverse client groups. Ability to work independently in community settings while maintaining collaborative relationships with interdisciplinary team members.

Excellent communication and relationship-building capabilities essential for building trust with vulnerable clients and their families.

Compensation :** Estimated annual earning potential ranges from $105,000 to $120,000, reflecting our commitment to competitive compensation for exceptional home healthcare professionals.
